Service Agreement

Clients are required to keep a Credit Card or Bank Account information on file prior to horse's Exam. Payment in full is required at time of service for all new clients.

Clients who currently have an IN-HOUSE account please use this time to update your credit card or bank information and sign the Service Agreement.

The following terms and conditions apply to all veterinary Services performed by Bur Oak Veterinary & Podiatry Services, LLC (BOVPS) for all horses of owner, whether individually identified on this document or not, unless the owner or authorized agent notifies BOVPS in writing that the Service Agreement does not apply to a designated horse(s), or this Service Agreement is replaced by a new Service Agreement.

1. This Service Agreement is in effect for all veterinary Services Performed by BOVPS on all patients owned/managed by me.

2. Payment options are: Cash, Check, Electronic Check, ACH, Domestic and International Wire (additional charges may apply), American Express, Master Card, Visa, Discover or PayPal. We do not accept Care Credit, Venmo or Zelle.

3. The credit card above will be charged at time of service for new clients.

4. If the current balance is not paid in 30 days, BOVPS is authorized to charge the credit card or bank account listed above for the entire balance owed.

5. Overdue accounts are subject to a service charge of 2.0% per month or 24% per annum. Accounts older than 90 days will receive no futher service until services are paid in full. After that those accounts will be required to send a deposit prior to work performed of $2,000-$5,000.

6. If there is any dispute over work performed, ownership, or billing, the client has 10 days from the statement date to reconcile the dispute with our office.

7. I hereby authorize BOVPS to provide routine care to my horse(s) in my abscence or at the request of my barn management. I acknowledge that BOVPS uses reasonable biosecurity measures and protocols to minimize any possible cross contamination while the horse is being treated. I hereby release and discharge BOVPS and its employees, agents, contractors, representatives, office staff, from any claims, demands, actions, liabilities, losses. injuries, illnesses, damages, costs or expenses (including without limitation attorneys' fees), known or unkown, fixed or contingent, arising out of or related to any infection or contageous disease or bodily injury of the owner's horse while under the care of BOVPS veterinarians, owners, directors and staff.

8. This contract shall apply to all veterinary services provided by BOVPS, to all horses on your behalf, whether or not horse(s) are listed on this form. Owner acknowledges and agrees that this Service Agreement shall continue year-to-year without need of a new document unless otherwise requested by Owner or BOVPS, in writing, or unless replaced by a new Service Agreement.

   
   

9. I agree to pay all costs and reasonable attorney fees incurred by BOVPS as a result of my default of this agreement, including the costs and fees of collection of any past due accounts.

By executing this agreement, the undersigned represents and warrants that he/she is the Owner of the horse(s) or is the Authorized Agent for the Owner with authority to execute this Agreement and bind the Owner to its Terms & Conditions.
